If you need a copy of the patient's physical or shot record, please contact the office for details.



If you are changing providers and want us to send records directly to your new provider, please fill out the form below and drop it off at our office. If you want a copy for your records, there is a fee of $1.00 per page for the first 25 pages and $.25 per page thereafter. Please allow a minimum of 72 hours for all records once a release is signed.
